We are proud to honour Veterans and their families at Worrigee Street Medical Centre.
We recognise the unique needs of Veterans and are proud to be able to give back in a tangible way to those who have selflessly served our great country. Our team consists of a few former ADF personnel and are able to provide a better understanding of some of the issues faced by our veterans.
We are committed to supporting our veterans to achieve good health outcomes by participating in the Coordinated Veterans Care (CVC) program. The CVC Program is a proactive coordinated care program. It aims to improve participant quality of life and decrease the risk of unplanned hospitalisation.
Within our general practice setting, the participant, their GP and a care coordinator work as a team to develop a care plan to:
- meet the health needs of the participant
- manage the participant’s ongoing care
The program promotes:
- health literacy
- self-management
- best practice coordination of care through a person-centred approach
Those eligible will be consulted by our nurse who performs the role of the care coordinator. Our medical team are focused on building positive relationships with our veterans and their families to promote their health and wellbeing.
